Prevalência de asma e rinite entre adolescentes de 13-14 anos em uma capital do Nordeste, de acordo com o questionário do International Study of Asthma and Allergies in Childhood (ISAAC)

Resumo
Objetivos: Determinar a prevalência de sintomas de asma e rinite, e avaliar a associação entre as duas doenças em adolescentes de 13-14 anos, estabelecendo comparações de gênero, tipo de escola e gravidade dos sintomas. Métodos: Estudo transversal, realizado com aplicação de questionários escritos envolvendo questões sobre sintomas de asma e rinite do International Study of Asthma and Allergies in Childhood (ISAAC) a 3.500 adolescentes de 13-14 anos na cidade de Maceió, Alagoas. A amostra foi obtida por meio de sorteio aleatório, respeitando a proporcionalidade entre escolas públicas e privadas em cada distrito da cidade. Para análise, 3.268 questionários foram considerados válidos. As comparações foram realizadas utilizando-se teste do Qui-quadrado. O nível de significância estabelecido foi de 5% (p < 0,05). Resultados: As prevalências encontradas foram de 13,2% para asma ativa, 32% para rinite ativa e 15,9% para rinoconjuntivite alérgica, com predominância de sintomas no gênero feminino. Doença grave foi relatada em 3,5% e 3,7% dos adolescentes para asma e rinite, respectivamente. Adolescentes de escolas privadas relataram mais frequentemente sintomas ativos e diagnóstico de asma e rinite. Considerando o total de adolescentes, 4,9% (162) apresentavam asma e rinite ativas concomitantemente. A frequência de rinite ativa entre asmáticos ativos e asmáticos graves foi significantemente maior quando comparada à frequência no grupo total de adolescentes (60% e 56% versus 32%, respectivamente, p < 0,01). Conclusões: A prevalência dos sintomas de asma e rinite situou-se dentro de valores intermediários, quando comparada à prevalência em outras áreas do mundo. Associação de sintomas de asma e rinite simultaneamente teve frequência comparável à média mundial, com aumento na prevalência de sintomas de rinite em asmáticos ativos e graves, em relação ao grupo total de adolescentes.

Objectives: To determine the prevalence of symptoms of asthma and rhinitis and to assess the association between the two conditions in adolescents aged 13-14 years, comparing gender, type of school and severity of symptoms. Methods: This cross-sectional study involved the application of written questionnaires covering symptoms of asthma and allergic rhinitis, namely, the International Study of Asthma and Allergies in Childhood (ISAAC) questionnaire, to 3,500 adolescents aged 13‑14 years in the city of Maceió, Alagoas, northeastern Brazil. The sample was obtained by random drawing, respecting the proportion of public and private schools in each city district. A total of 3,268 questionnaires were considered valid for the analysis. Comparisons were performed using the chi- square test. Significance was set at 5% (p < 0.05). Results: A prevalence of 13.2% was found for active asthma, 32% for active rhinitis, and 15.9% for allergic rhinoconjunctivitis, with a predominance of symptoms among females. Severe disease was reported by 3.5 and 3.7% of the adolescents with asthma and rhinitis, respectively. Adolescents from private schools more frequently reported active symptoms and a diagnosis of asthma and rhinitis. Considering the total sample, 4.9% (162) presented active asthma and rhinitis concurrently. The frequency of active rhinitis among active asthmatics and among severe asthmatics was significantly higher when compared to the frequency in the total sample of adolescents (60 and 56% vs. 32%, respectively; p < 0.01). Conclusions: The prevalence of symptoms of asthma and rhinitis in our sample was within intermediate values in comparison with prevalence rates reported for other parts of the world. The simultaneous presence of asthma and rhinitis symptoms showed a frequency compatible with international mean values, with an increased prevalence of symptoms of rhinitis in active and severe asthmatics when compared to the whole sample.
